NS	Parent Contract (signed by BOTH parents)
US Club	2009/2010 US CLUB Registration Form
MSPSP	Michigan State Premier Soccer League Player Registration Form
NS	Reserve Player Agreement ( Reserves Only)
Print ALL (this will print all the above forms needed to register )

Player Registration

***Parents and Players must attend registration. Exceptions can only be approved by the coach.***

Also known as the Triple “P” Evening … paperwork/pictures/payments …You will need to provide the following:

______ Bring with you 2 copies of player Birth Certificate

Items to be turned in and finalized during this registration process can be completed ahead of time

______Signed Parent Contract (last page only) NO EXCEPTIONS

______NorthStar Medical Release form. A notary will be present Registration Night to notarize your

Signature...remember to sign this form at registration in the presence of the notary.

EVERYONE needs to complete this form each year.

______ US Club Registration form for players new to NorthStar

______ Completion of MSPSP (Michigan State Premier Soccer Program) Player Registration form

______ Photos for player pass cards will be taken of each player in a NorthStar jersey

______ NorthStar League/Club/Team Check (if not paid by ccard online) fee --payable to your NorthStar Soccer Club S ee team plan for specific amount.

______ Uniform order form with check payable to NorthStar Soccer Club, if needed

______ Optional: NorthStar logowear form with check payable to: NorthStar Soccer Inc.

Logo wear items ordered today will be delivered at the NorthStar Picnic in August

Please be prepared to write up to 3 checks during this process.
